A sonohysterogram, also called saline infusion sonography, is a minimally invasive procedure used to evaluate the uterine cavity and detect potential concerns such as abnormal growths or fertility issues. During the procedure, a doctor performs a gentle insertion of a thin tube through the cervical canal, through which sterile fluid is introduced to expand the uterus for better imaging. An ultrasound transducer, often referred to as an ultrasound wand, is then used to perform an ultrasound exam to visualize the internal structures.
In some cases, Doppler ultrasound may be used to assess blood flow and identify abnormalities that require further evaluation. This test offers a clearer look at the internal organs, especially when standard other tests may not provide enough detail. While most women tolerate the procedure well, some may experience mild cramping or spotting; however, intense pain or heavy vaginal bleeding is uncommon and should be reported. After the procedure, patients can usually resume their normal activities without downtime.
What is a Sonohysterogram?
A sonohysterogram is an imaging test using ultrasound performed to look inside the uterus. Ultrasound imaging is a non-invasive and safe test that helps your doctor visualize internal structures and organs. You may be advised to have this test if you have symptoms that are associated with uterine problems, such as abnormal menstrual bleeding or cramps, repeated miscarriages, infertility, an abnormal pelvic exam, post-menopausal bleeding, or more.

A sonohysterogram is typically recommended for individuals with abnormal uterine bleeding, infertility, recurrent miscarriage, or suspected uterine abnormalities. Candidates should not be pregnant or have an active pelvic infection, and are usually scheduled for the test early in their menstrual cycle, after menstruation but before ovulation, for the clearest and safest results.
Preparing for The Sonohysterogram Procedure
Prior to your sonohysterogram test, you will be asked to take a pregnancy test, as pregnant women should not undergo this procedure. This test is usually done one week after your period starts, as this is when your uterine lining is thinnest, allowing your doctor to view results more easily. You will also be asked to use the restroom to empty your bladder before the test.

During the Sonohysterogram Procedure
Overall, the sonohysterogram procedure can be broken down into the following steps:
- Second transvaginal ultrasound: In the last step of this test, the transvaginal ultrasound probe will be reinserted and more fluid will be sent through the inserted tube to the uterus. This ultrasound will be used to examine the uterine lining and note the flow of fluid from the uterus into the fallopian tubes. This exam takes about 30 minutes to complete.
- Initial transvaginal ultrasound: To begin, you will lie down on the exam table and an initial transvaginal ultrasound will be performed. This involves the insertion of an ultrasound probe into the vagina, which will emit sound waves to create an image of your uterine lining.
- Inserting fluid into the uterus: Next, a tool called a speculum will be inserted into the vagina to keep it open, making it easier for your doctor to reach the cervix. A special swab will be used to clean the inside of the cervix, and a tube will be inserted to send fluid into the cervical opening. This fluid will cause your uterus to get slightly larger and make the uterine lining easier to see.
